This was a very popular Dragon Quest and was very difficult to get.. I heard it sold 500,000 copies very quickly and that one person actually got mobbed for it because the lines were so long.

| You start out with 2 Clubs, Cypress Stick, and Wayfarer's Clothes | |
| Once again, religious content was censored for the U.S. release. The priest looks different. The Houses of Healing had a Christian cross instead of a Jewish star. The pilgrim character class even looked different. The shrines also resembled Japanese Temples. | |
| On a similar note, in the original Japanese version, your dead companions actually turned into coffins you pulled along behind you. For some reason this was changed so that your dead companions turned into ghosts that looked like Marshmallow Men. Guess that was too scary for American audiances ;) | |
| There were three items left out of the U.S. version of the game. One was the Sword Horned. This sword could only be weilded by a soldier. It was more powerful than the Sword of Kings, but carried a curse with it as well. Another was the Slime Stick. The final missing item was the Black Raven. This was a golden statue worth 55,000 gold. | |
